Advanced Orthopaedic Care for Hip and Knee Conditions

At Northampton Hip and Knee Clinic, our specialist consultant orthopaedic surgeon, Mr Pierre Nasr, provides a comprehensive service for the diagnosis and management of trauma, orthopaedic, and sports injury-related hip and knee problems. With many years of experience and a commitment to personalised care, we offer cutting-edge treatments that have been clinically proven through the national joint registry data. This will help you regain mobility, reduce pain, and improve your quality of life.

Whilst many patients visit their General Practitioner with a history of hip or knee pain, Mr Nasr can offer swift access to all that a patient may require, from consultations, x rays, CT and MRI scanning to injections and surgery. Clinics are carried out both during the daytime but also into the evenings, on a weekly basis.

    Hip Replacement

    A proven approach

Our hip replacement surgery is designed to alleviate pain and restore function to allow you to have a better quality of life. During this procedure, we carefully remove the damaged arthritic top part of the thigh bone (femur) where the ball joint is and also take away the damaged part of the hip socket (acetabulum). This can be done via a relatively small incision near the back of the hip, towards the buttock, where scars are often hidden behind underwear. Mr Nasr uses the well established posterior approach to a hip replacement. There are other alternatives such as the anterior approach to the hip.

These are then replaced with state-of-the-art metal components, with high quality bearing surfaces placed between them to ensure smooth, pain-free movement. These can include bearings such as ceramic, high density polyethylene (a form of plastic), stainless steel or alloys, such as cobalt chrome. These advanced techniques allow for improved mobility and a significant reduction in pain, helping you return to your daily activities with renewed comfort.

Driven by data and clinical evidence:

The National Joint Registry (NJR) collects and published data on all joint replacements in the England and Wales, Northern Ireland, the Isle of Man and Guernsey. The largest orthopaedic registry in the world allows surgeons to scrutinise the data on the performance of the implants that they chose for their patients.

Hip resurfacing ‘ The Andy Murray operation’

Another alternative is a hip resurfacing replacement. This is reserved for younger, more active patients. The data regarding this type of joint replacement is that it is suitable for men, as women had a higher complication rate. The tennis player, Andy Murray, underwent this procedure, which allowed him to get back to professional sport. The advantages of this type of joint replacement are better function, better range of motion, lower risk of revision surgery whilst being more bone conserving on the femoral side if a revision procedure is required decades down the line.

    Revision hip surgery:

We also specialise in hip revision surgery which involves surgery to remove a damaged, worn out, loose or broken hip replacement. Hip replacements do last a long time but as many have been inserted a long time ago, there are now an increasing amount that need attention to be able to give patients a pain free and well functioning revision hip replacement. Mr Nasr is one of a few that specialise in revision hip surgery in Northamptonshire and is happy to take referrals from further afield. Previous patients have come to the clinic from London, Buckinghamshire, Bedfordshire, Oxfordshire, Cambridgeshire and Leicestershire.

    Knee Replacement

Similar to hip replacement, our knee replacement surgery focuses on removing the damaged, arthritic surfaces of the thigh bone (femur) and shin bone (tibia). These are replaced with precisely fitted metal components, with a high-density polyethylene plastic insert placed between them. This creates a new, smoothly functioning joint that reduces pain and improves mobility. Our surgeon’s expertise ensures optimal alignment and balance, crucial for the long-term success of your new knee.

    Partial knee replacement

Partial knee replacement is the only minimally invasive option for knee replacement surgery. With partial knee replacement, only the damaged parts of your knee are replaced, thus retaining all the other normal bone, cartilage, and ligaments in your knee. There are 3 types of partial knee replacements: medial (the inside of your knee – closest to the other knee), lateral (the outside of your knee), and patellofemoral (the front of your knee, often referred to as the kneecap). A medial partial knee replacement is the most commonly performed. With these procedures only the damaged compartment of the knee is replaced. As a result, patients are often able to recover more quickly and have a better level of function. A partial knee replacement is a successful long-term option in appropriately selected patients.

Unfortunately, not all patients are suitable for partial knee replacement.

    Total knee replacement

Total knee replacement may be necessary when several portions of the knee are damaged by arthritis. During a total knee replacement, the arthritic portions of the femur and tibia are removed and replaced with metal implants. Then, a medical grade polyethylene plastic implant is inserted between the two that will create the new smooth bearing surface that should last for decades.

Hip and Knee Pain Relieving Injections

For some patients, injections can provide significant relief from joint pain and inflammation. We offer various types of injections, including corticosteroids and hyaluronic acid, tailored to your specific condition. These minimally invasive treatments can offer quick relief and are often used as part of a comprehensive treatment plan.

Some patients may not be ready for surgery as they have yet to develop severe arthritis. These patients may be suitable for recurrent injections over many years, which may delay surgery if they are young or if they are not keen on pursuing a surgical option.

Some patients might be a bit too frail to undergo surgery and injections can be an option to control pain.

Pierre Nasr Hip and Knee Injections (youtube.com)

Hip joint injections can be directed into a hip joint using x-ray or ultrasound guidance and will need to be booked after your initial consultation. They are performed in our minor procedure room or in our award winning imaging department by Consultant radiologist who specialise in Musculo-Skeletal condition.

Knee joint injections can sometimes be performed on the same day on the initial consultation if appropriate.

Mr Nasr also offers injections for pain relief of other hip and knee conditions, such as bursitis, inflammatory and degenerative tendon issues.

    Hyaluronic acid injections

Hyaluronic acid is a naturally occurring component of joints and synthetic versions have been manufactured over the years to help patients control their joint symptoms. The injections aim to reduce inflammation, swelling and improve pain to allow you to get back your movement and quality of life.

All injections should give relief straight away due to the local anaesthetic that is used with these injections but they sometimes take a day or so to start to have an effect. The effect of these injections may be anywhere from a couple of months to over 6 months and patients are able to have up to 2 injections per year, which can provide the relief that you are looking for in order to avoid surgery.

If arthritis is too far advanced, then injections will not give you an acceptable level of benefit and surgery is, therefore, the last remaining option.

    Hip Arthritis

The hip joint is a ball and socket joint. It usually allows smooth movement to occur between the surfaces and unrestricted, pain free movement. However, when the smooth cartilage wears away the joint starts to malfunction and will ultimately lead to arthritis.

Arthritis in the hip can be debilitating, affecting your ability to walk, work, and enjoy life. We specialise in treating both osteoarthritis (wear-and-tear arthritis) and inflammatory arthritis such as rheumatoid arthritis (an autoimmune condition) of the hip. Our treatments range from conservative management, advice on pain coping strategies, joint injections as well as discussions surrounding the risks and benefits of joint replacement surgery.

Arthritis can be considered to be a process by which the joint has failed which causes pain, stiffness, weakness, deformity and disability. Arthritis can have a significant impact on a patient’s quality of life affecting not just their physical health but emotional wellbeing and relationships with friends and family.

    Knee Arthritis

Whether you’re dealing with osteoarthritis or rheumatoid arthritis in your knee, our clinic offers a comprehensive and full range of treatments to manage pain and improve function. Mr Nasr is able to diagnose and advise on all aspects of knee pain.

We begin with conservative approaches and progress to more advanced treatments as needed, always focusing on your individual needs and goals.
The knee is comprised of various compartments which can wear at different rates

    1. The articulation between the shin bone and the thigh bone on the inside and another on the outside.
    2. The kneecap joint where the undersurface of the knee cap articulates with the bottom end of the thigh bone.

As well as bone and cartilage, the knee also consists of muscles, ligaments and tendons. There are also bursae which are fluid filled sacs that can become inflamed.

    The Meniscus

This is a rubbery, crescent-shaped section of cartilage around the edge of the shin bone that fills the space between thigh bone and shin bone. They help to absorb shock, spreading stress around the joint, whilst also providing a soft, smooth surface to protect the articular cartilage from wear and tear. Sometimes, these get damaged and if not related to significant arthritis can be trimmed via keyhole minimally invasive surgery.

    Arthroscopy (Keyhole Surgery)

Our arthroscopic procedures allow us to diagnose and treat various joint conditions through small incisions. This minimally invasive approach is particularly effective for knee conditions such as meniscal tears and ligament injuries. Benefits include less pain, faster recovery, and minimal scarring compared to traditional open surgery.
Mr Nasr does not currently perform Anterior Cruciate Ligament (ACL) surgery, as he concentrates on surgery for meniscal tears and joint replacement surgery.
